What is GP-Centered Care?

GP-Centered Care — also known as the “Family Doctor Model” — is a special healthcare program that focuses your medical care around one primary practice: your family doctor’s office.

In practical terms, this means you come to us first for any health concerns.
We provide personal, continuous, and well-coordinated care, referring you to specialists or hospitals when needed — while keeping an overview of your entire treatment journey.

Your Benefits

  • Better care: We have a complete view of your medical history, treatments, and medications — making your care safer and more effective.

  • Fewer duplicate tests: With centralized coordination, you avoid unnecessary repeat examinations and save time.

  • Shorter waiting times: Many HzV contracts include faster access to specialist appointments, especially for chronic conditions.

  • Regular preventive check-ups: You receive structured programs for early detection and better management of chronic diseases (e.g. diabetes, high blood pressure).

  • More time for you: Because GP-Centered Care is better compensated, we can spend more time on counseling, questions, and your overall health needs.

Are there any downsides?

Just one — and it’s actually an advantage:

  • By enrolling, you agree to visit our practice first before seeing a specialist (exceptions: gynecology, ophthalmology, and emergencies).

This ensures that you always have someone coordinating your care and keeping track of the big picture

Why we recommend joining

Weil gute Medizin Zeit und Überblick braucht.

Good medicine takes time, continuity, and a clear overview. With GP-Centered Care, we can support you more personally, comprehensively, and efficiently. We can better manage your medications, detect side effects early, interpret specialist reports accurately, and guide you safely through the healthcare system.

In short: By enrolling in the HzV program, you help us provide even better care for you.

How to enroll

It’s simple: Just talk to us during your next visit — we’ll answer your questions and take care of the registration process for you.
